Our Facility
Summit Commons is a 130-bed skilled nursing facility staffed with healthcare professionals who cater to the needs of our residents. Our facility offers Transitional Care, Skilled Nursing, Specialized Care for Alzheimer's and other memory related impairments, Long-Term Care, Hospice and Palliative Care Services.
Summit Commons provides a continuum of healthcare services not only to the residents of Providence, but also to many other surrounding cities. Our facility is within close proximity to physicians’ offices and is minutes away from all major Providence hospitals.
At Summit Commons, residents can choose from a selection of accommodations. All rooms are completely furnished, but residents are encouraged to add their own personal touches.

Our Services
We are certified by Medicare and Medicaid.
Summit Commons accepts admissions 24 hours a day, seven days a week. We care for short-stay post-acute patients as well as long-term care residents. Our physical therapy gym provides ample room for rehabilitation activities.
Our Short Stay, Transitional Care program features 7 day-a-week rehabilitation services, complex nursing care when needed and includes full case management, and complete discharge planning.
Our Rehabilitative Capabilities
Our Rehabilitation Programs at Summit Commons use a comprehensive team approach with each specialist contributing to an aggressive treatment plan. The goal for our patients is to help restore maximum function and independence, and to help in maintaining that independence by focusing on skills training needed for their return home.
Our Transitional Care program’s goal is to return residents to their highest level of functioning with the ultimate aim of discharge to home. For that reason family involvement in the care plan process is encouraged. Whether the stay at Summit Commons is considered short or long-term, the patient’s emotional well-being enhances successful outcomes.
