Radius provides a team of qualified professionals and volunteers to work in cooperation to meet the physical, emotional, social and spiritual needs of our patients.
Along with this we offer the support and guidance needed by family members and loved ones at this difficult crossroad in their life and the life of their loved one.
- Primary Care Physician - Becomes an integral part of the hospice team and continues to direct the plan of care.
- Hospice Medical Director - Oversees the medical services provided to each patient in the hospice program.
- Licensed Nurses - Specialize in pain management and symptom control for end stage disease.
- Certified Home Health Aides - Provide personal care and help with the activities of daily living
- Licensed Social Workers - Assist the patient and family with emotional concerns, including bereavement, as well as access to support groups.
- Pastoral Counselors - Support the family and loved ones of all ages with support groups, one-on-one counseling and other special services during the bereavement process.
- Rehabilitation Therapists - Assist in helping to improve the patient's quality of life.
- Volunteers - Have been trained to provide the companionship and support that the patient and family may need with the everyday challenges of life.
